Transcripts

Transcript requests can be submitted via email or by contacting Mrs. Charvat in the counseling office. Please send an email to lcharvat@smokyvalley.org with the following information:

• Name (current and maiden if applicable)
• Year of Graduation
• Where the transcript needs to be sent (both the name of the institution and the address if possible)

Transcripts will be processed within 48 business hours of the request.
